Management of Patients with COVID during the Public Health Emergency in India April 30, 2021

COVID symptoms: Fever, cough, shortness of breath, headache, body ache, loss of taste or smell 

If symptoms present, should be triaged as below, regardless of test results


Definition

Mild COVID
Can Talk AND Can Walk 

  1. Talk
  2. Walk

Moderate COVID
Can Talk but CAN’T WALK

Severe COVID
Can’t Talk, Can’t Walk

Management

  • Stay home, do not go out
  • Wear a mask 
  • Fluids (oral) 
  • Paracetamol
  • Reassure
  • Provide homecare sheet (link to IndiaCovid SOS)

Admit to hospital if bed available

  • Supplemental oxygen to keep comfortable or sats >90% if pulse oximeter available
  • Awake proning 
  • Dexamethasone 6 mg day or other steroid equivalent* for 5 days 
  • Apixaban 2.5 mg twice a day oral if available

Admit to hospital

  • Supplemental oxygen to keep comfortable or sats >90% if pulse oximeter available
  • Awake proning 
  • Dexamethasone 6 mg day
  • If worsens despite above treatment 
    • Noninvasive ventilation
    • Mechanical ventilation
    • ONLY in Age < 60 yrs and no h/o diabetes increase Dexamethasone dose to 20 mg  once a day 

Labs / Radiology

No blood tests

CBC, Creatinine, CRP, d-dimer at admission if possible

Radiology

No Chest Xray, No CT scan

No Chest Xray, no CT scan

No routine chest Xray or CT scan

Steroids equivalent to Dexamethasone 6 mg/once a day include: 

Prednisolone 20 mg twice a day for 5 days
OR
Prednisone 20 mg twice a day for 5 days
OR
Hydrocortisone 40 mg given 4 times/day for 5 days 
OR 
Methylprednisolone 16 mg twice a day for 5 days
